Hazardous Occurrence Reporting

  1. All accidents, hazardous exposures, ergonomic injuries and workplace illnesses on DND property must be reported. This includes accidents with property damage only (no injury to personnel).
  2. All accidents, hazardous exposures and ergonomic injuries to DND civilian employees or CF members while on-duty or authorized sport/physical training must be reported even if they occur outside DND property.
  3. All disabling injuries or accidental deaths to off-duty CF members shall be reported, including those outside DND property.
  4. In addition, people are encouraged to report and investigate work-related incidents and hazardous conditions that did not result in damage or injury.
